Personality disorders affect a person’s thoughts, behaviors, and feelings, often leading to strain or stress in their personal and professional lives. Personality disorders can cause disorganized thinking and erratic behaviors. Unfortunately, these kinds of disorders can be difficult to diagnose since people with personality disorders often do not realize something is wrong with their behavior. 

Each personality disorder belongs to a cluster (A, B, or C) and each cluster is further categorized by different traits. Three out of ten personality disorders are cluster C personality disorders, which are commonly known to show symptoms based in fear and anxiety.

What Is a Cluster C Personality Disorder?

Unlike cluster A and cluster B personality disorders, cluster C personality disorders are primarily characterized by traits of behaviors rooted in fear, anxiety, and insecurity [1]. 

What Are Cluster C Personality Disorders?

There are ten known personality disorders as characterized by the Diagnostic and Statistical Manual of Mental Disorders. Of these ten, three are cluster C personality disorders, which are the following:

Avoidant Personality Disorder

Also known as AVPD, avoidant personality disorder affects those who have it by instilling the belief that they are unworthy and inferior. This belief stems outwardly, and people who have AVPD also believe that everyone else knows and thinks they are inferior. This then causes an extreme fear of being negatively evaluated or perceived by others, even though the affected person longs for connection. This also causes an extreme sensitivity to any rejection or criticism.

These behaviors and thought patterns present themselves in the following symptoms:

  • Avoiding most or all social interactions
  • Avoiding situations that expose them to potential disapproval
  • Avoiding new things or new places because it means they may have to interact with people.

Those struggling with avoidant personality disorder often suppress their true beliefs, feelings, or emotions. AVPD has similarities to social anxiety; some mental health experts believe that avoidant personality disorder is a more severe form of social anxiety rather than its own condition.

Dependent Personality Disorder

Individuals who struggle with dependent personality disorder have an extreme fear of separation or being left alone. This leads to them solely depending on others [2] and wanting others to take care of them. Dependent personality disorder can manifest in symptoms like:

  • The difficulty of making decisions without others’ approval or opinion
  • Extreme submissiveness
  • Constant need for validation, approval, or reassurance
  • Seeking an abnormal amount of emotional or practical support from others
  • Difficulty expressing when they don’t like something or when something bothers them
  • Avoiding conflict and disagreement
  • Feeling uncomfortable or anxious when they are alone
  • An intense fear of abandonment

People with dependent personality disorder may come off as excessively clingy and experience struggles with functioning independently.

Obsessive Compulsive Personality Disorder

Obsessive-compulsive disorder is a chronic disorder in which an individual experiences obsessions and engages in compulsions [3]. People with OCD experience uncontrollable, recurring thoughts, engage in repetitive behaviors, or both. These compulsions and obsessions can take a heavy toll on someone’s life, since the symptoms are time-consuming and interfere with daily functions. 

The diagnostic criteria for OCD involve people who spend more than one hour a day on their obsessions and compulsions; people who cannot control recurring thoughts or behaviors even though they know it may be excessive; and who do not get any pleasure from the compulsions but may feel temporary anxiety. 

What Are the Traits of Cluster C Personality Disorders?

Cluster C personality disorders are highly anxious and fearful, but each disorder has its own set of symptoms and unique variations.

Here are common symptoms observed across the three cluster C personality disorders:

  • An abnormal amount of anxiety and fear in regular situations and everyday interactions
  • Avoidance to situations that could lead to embarrassment or rejection
  • Perfectionist tendencies and striving for unrealistic standards

How Cluster C Differs from Other Disorders

Cluster C personality disorders involve anxious, fearful behavior driven by chronic insecurity and a strong need for reassurance. To better understand Cluster C, it’s helpful to compare it to the other personality disorder clusters.

Personality Disorders in cluster A are marked by odd, eccentric behavior or beliefs, and cognitive distortions. This includes Schizoid Personality Disorder, Schizotypal Personality Disorder, and Paranoid Personality Disorder.

Personality Disorders in cluster B involve erratic and melodramatic behavior patterns. Disorders in this group include Antisocial Personality Disorder, Histrionic Personality Disorder, Borderline Personality Disorder, and Narcissistic Personality Disorder.

Diagnosing Cluster C Personality Disorders

Diagnosing cluster C personality disorders can be challenging since individuals with these disorders often have difficulty recognizing their symptoms as problematic. This mindset can lead to a reluctance to seek treatment or avoidance of help, even when it is offered.

Mental health professionals typically evaluate Cluster C disorders by exploring your personal history, personality traits, coping mechanisms, and relationships. They may also assess for any co-occurring mental health conditions, such as depression or anxiety.

Treating Cluster C Personality Disorders

Treatment for cluster C personality disorders typically involves a combination of medication and psychotherapy.

Medication for Cluster C Personality Disorders

While no specific medications are approved for treating personality disorders, some mental health professionals may prescribe medications to help manage associated symptoms. These can include antidepressants, anti-anxiety medications, mood stabilizers, or antipsychotics.

Psychotherapy

Psychotherapy, or talk therapy, involves regular sessions with a therapist to discuss thoughts, feelings, and behaviors. Various types of psychotherapy that can be effective for cluster C personality disorders include:

  • Cognitive Behavioral Therapy (CBT)
  • Dialectical Behavior Therapy (DBT)
  • Psychoanalytic Therapy
  • Psychoeducation

The primary aim of talk therapy is to equip individuals with cluster C personality disorders with tools to enhance their quality of life. This includes identifying personal challenges, learning from experiences, and gaining awareness of emotional distress.

How to Help Someone Who Has a Cluster C Personality Disorder

Helping someone with a cluster C personality disorder can be difficult, whether they have or have not yet been diagnosed. If you suspect someone you love has a cluster C personality disorder, you can gently suggest they see their primary care physician, who can then refer them to a psychiatrist. Some folks are more likely to follow a doctor’s advice rather than that of a friend. 

If you have someone in your life who has a diagnosed cluster C personality disorder, you can support them by: 

  • Prioritize your own well-being. Supporting someone else is only sustainable when you’re caring for yourself, too.
  • Give them space to grow. Recovery takes time, and patience helps build trust along the way.
  • Don’t internalize their actions. Their behaviors often stem from the disorder, not from anything you’ve done.
  • Speak from your own experience. Using “I” statements can help keep conversations grounded and respectful.
  • Recognize their progress. Even small steps forward deserve acknowledgment and encouragement.
  • Lend a hand where you can. Offering help with logistics, like rides to therapy or managing appointments, can make a real difference.
  • Be willing to sit beside them in therapy. If they’re open to it, your presence in a session can show support and deepen understanding.
